Description: As the landscape of health care continues to evolve at a rapid pace, we must be sure that we are applying the evidence that helps support patient-centered care. Health care organizations are now in the transition from fee-based to value-based care. Data and evidence are now part of our daily care routines and we can use these tools to help us achieve the Triple Aim. According to the Institute of Health Care Improvement ”improving the US health care system requires simultaneous pursuit of three aims: improving the patient experience of care (including quality and satisfaction); improving the health of populations; and reducing the per capita costs of health care."

At the completion of this activity, participants will be able to:

  • State prominent reasons for frequent readmissions.
  • Identify social disparities that contribute to frequent readmissions.
  • Explain concepts to reducing readmissions.
  • Formulate a strategy to reduce readmission and prevent admissions.
