Description: This program will help health professionals conduct effective transition planning. Participants will learn how to assess when a transition to a higher level of care is necessary and become familiar with the tools and resources available to assist this process.

At the completion of this activity, participants will be able to:

  • Identify the needs of aging Americans.
  • Discuss perspectives on the available resources to seniors.
  • Collect information on how to appropriately assess seniors for transitioning into a higher level of care
