Yolanda Hughes
Yolanda Hughes is the Home Again Program Manager in the Office of Waiver and Facility Services at the South Carolina Department of Health and Human Services. Through the Home Again Program, she and her team help eligible individuals transition from long-term care institutions (skilled nursing facilities or hospitals) back to community living. Yolanda collaboratively identifies and implements training and growth opportunities for her team to ensure program success, manages a schedule of maintenance and review of policy/provider manuals, bulletins containing program guidelines, training curricula and provider credentialing requirements. As the Program Manager, she executes and implements federal budgets for the Home Again Grant program in accordance with Centers for Medicare and Medicaid Services (CMS) guidelines. Throughout her career she has held positions at the SC State Housing Finance and Development Authority, SC Department of Public Health and the SC Department of Social Services. She holds bachelor’s degree in Healthcare Management from Clayton State University.
