Description
The purpose of this presentation is to share effort in the local, tri-county area (Charleston, Dorchester and Berkeley counties) to meet goals of a collaborative, Tri-County Health Improvement Plan (TCHIP) that specifically seeks to improve maternal mortality and morbidity in African Americans. This module contains a recorded presentation and evaluation.
Objectives

At the completion of this activity, participants will be able to:

  • Be familiar with the process of undertaking a population health initiative at the local level.
  • Appreciate the value of engaging the involvement of a wide variety of community partners who take ownership in implementing the planning objectives and action plans.
  • Identify, by example, the benefit of drawing upon others involved in national and statewide priority-setting, data collection, and solutions for both metrics and outcomes at the local level.
  • View population health as a dynamic and interdisciplinary approach to marshall the resources needed to assure – and sustain – community wellness and equity at the local level.
