Description
This program highlights the way remote patient monitoring programs for diabetes can assist practices with the management of diabetes. Telehealth applications specific to medication management are presented. This program discusses the use of telehealth applications within the home setting to better address chronic disease management issues. Examples of SC programs designed to address health disparities in rural and underserved communities are provided. Support for this program was provided by a HRSA Network Development Grant in partnership with Palmetto Care Connections.

At the completion of this activity, participants will be able to:

  • Develop an increased knowledge of medications for diabetes
  • Design a medication plan for a patient with newly diagnosed diabetes
  • Initiate an insulin regimen for a patient previously on oral diabetic medications
