Description
The first-ever South Carolina Population Health Summit was held May 20, 2016. The Summit brought together people who are grassroots community leaders, along with people who run organizations that directly impact the health and economic well-being of each community, to have a conversation about what next steps are needed to improve the health of the populations they serve.
Objectives

At the completion of this activity, participants will be able to:

  • Learn practical advice on how to move from data analysis to action to improve population health outcomes. 
  • Improve their knowledge of regional assets and leverage opportunities to accelerate population health outcome improvement in the region. 
  • Learn practical advice on how to keep momentum of a population health improvement coalition in order to impact outcomes.
  • Identify opportunities to collaborate to maximize the impact of existing regional assets.
  • Define at least one tangible next step to apply the knowledge gained in the summit.
  • Learn about the 5 regional commitments reached during the day, and reflect on how they can catalyze population health improvements in their regions.
