Description
The primary objective of the Summit is to educate state and community leaders on how to move from data analysis to action to improve population health outcomes. Secondary objectives include improving participants' knowledge of regional assets and identifying opportunities to collaborate and maximize the impact of those assets. Finally, facilitators at each regional site will lead discussions to set goals for the coming year. Facilitators and participants will use state and local health assessments as a guide for this goal-setting exercise. Given that South Carolina is one of the 10 least healthy states according to America's Health Rankings, our Summit can also make use of the Roadmaps to Health Action Center tools and resources as a guide for this goal setting.
Objectives

At the completion of this activity, participants will be able to:

  • Describe the differences between personal health, population health, and public health.
  • Utilize population health in common community health initiatives
  • Explain the interface between population health and public health
  • Engage in collaboration between the public and private sectors in improving community health and wellbeing
  • Define health equity, health inequalities, and social determinants of health
  • Describe how they would leverage partnerships and cross sector collaborations to advance health equity
  • Implement at least two methods or strategies to increase health equity in local communities
  • Describe at least one strategy that they would use to move from data to action in partnership with their community
  • Utilize data as a starting point for identifying opportunities in their community
  • Identify health factors for their county that may have the greatest potential opportunity for improvement
  • Describe how systems change and innovation are necessary approaches to solving complex social problems
  • Identify the five key elements of collective impact: common agenda, common progress measures, mutually reinforcing activities, communications, and a backbone organization
  • Implement two strategies to engage partners from different sectors to tackle complex social problems
