Description
The SC Provider Toolkit was developed to fulfill a recognized need for health care providers to better manage their patients with prediabetes. It is a resource that health care teams may use as a guide to evaluate, test, treat and refer patients to in-person or online evidence-based diabetes prevention programs. The goal is to increase the number of physicians, health care providers and healthcare consumers that recognize prediabetes as a treatable condition.
Objectives
At the completion of this activity, participants will be able to:
Define prediabetes and identify common risk factors for both prediabetes and diabetes
Be introduced and learn how to use the SC Provider Toolkit which can be incorporated into their practice.
Learn about the CDC National Diabetes Prevention Program.
Evaluate, test, treat and refer patients to the evidence based program (NDPP) using the SC Provider Toolkit guide
