Description
South Carolina Chronic Conditions Care Collaborative (4C Collaborative) helps support health systems across the state of South Carolina to improve patient outcomes. In this course, 4C Collaborative provides evidence-based strategies and resources to assist health care professionals in the management of patients with chronic diseases. Topics covered in the course include: implicit bias, identifying undiagnosed hypertension, medication therapy management, team-based care, and clinical systems and care practices to improve diabetes care.
Objectives

At the completion of this activity, participants will be able to:

  • Utilize the 4C Collaborative Change Package to improve health outcomes for patients with chronic conditions.
  • Discuss examples of change concepts, change ideas, and evidence-based resources for the improvement of patient care and health outcomes for patients with chronic conditions.
  • Describe the use of quality improvement methodologies (i.e., Plan-Do-Study-Act cycles) to test and implement new processes and/or strategies to improve health outcomes related to chronic conditions.
  • Identify approaches to incorporate non-physician team members in hypertension and cholesterol management in the clinical setting.
  • Explain the role of implicit bias in health care disparities and how to recognize these biases to address disparities at the practice level.
