Description
The COVID-19 pandemic was an unprecedented challenge for healthcare professionals and communities. This webinar will focus on the lessons learned, including challenges with healthcare access for rural South Carolinians, historical and contemporary mistrust of the medical system, the need for robust individual and family care during care transitions (e.g., hospital to home-based care), and the importance of social connections. We will highlight innovative models of care and discuss opportunities to support populations, such as young adults, that may have unique health challenges related to the pandemic.

Objectives

At the completion of this activity, participants will be able to:

  • Analyze the COVID-19 related challenges faced by rural and marginalized patients and families in rural South Carolina, focusing on healthcare access and transition care.
  • Describe the impact of the COVID-19 pandemic on social needs, family and social relationships, and related health implications.
  • Discuss innovative models of care to support patients and families during care transitions.

This program is supported by funding from the Health Equity Initiative, in partnership with the SC Office of Rural Health (SCORH) and SC Department of Health and Environmental Control (SC DHEC). The Health Equity Initiative is part of a national project by the Centers for Disease Control and Prevention (CDC), supported by funding through the Coronavirus Response and Relief Supplemental Appropriations Act. Lowcountry AHEC Disclosure Statement Certificate of attendance A certificate of attendance will be awarded to participants who have completed the evaluation and post assessment requirements. Participants MUST attend 90% of the program to receive the allotted contact hours for that program. Partial credit will not be given.
