Description
We will be taking time to identify the Who, What, Where, When and Why of the patient’s goals and how to support the patient and family in the process of teasing them out. We will also take a look at when professional opinions may differ from family and when to re-address goals as needs change.
Objectives

At the completion of this activity, participants will be able to:

  • Identify why we have this conversation.
  • Identify potential conflicts or obstacles.
  • Identify when the goals may need to be re-addressed and how to approach.
