Description
This session will equip Community Health Workers (CHWs) with a clear understanding of the key medical and legal documents that help clients communicate their healthcare wishes and ensure those wishes are respected. Participants will learn how these documents can empower individuals to designate a trusted representative to speak on their behalf and how to identify which form is most appropriate for different situations. By strengthening their knowledge in this area, CHWs can better support clients in making informed decisions about their care and advocacy.
Objectives

At the completion of this activity, participants will be able to:

  • Identify various documents available to clients to make their healthcare wishes known
  • Demonstrate an understanding of which document empowers an agent to speak on the clients half.
  • Differentiate between the various documents and select which is appropriate for a given situation.
