Description
The purpose of this presentation is to share effort in the local, tri-county area (Charleston, Dorchester and Berkeley counties) to meet goals of a collaborative, Tri-County Health Improvement Plan (TCHIP) that specifically seeks to improve maternal mortality and morbidity in African Americans. This module contains a recorded presentation and evaluation.
Objectives
At the completion of this activity, participants will be able to:
Be familiar with the process of undertaking a population health initiative at the local level.
Appreciate the value of engaging the involvement of a wide variety of community partners who take ownership in implementing the planning objectives and action plans.
Identify, by example, the benefit of drawing upon others involved in national and statewide priority-setting, data collection, and solutions for both metrics and outcomes at the local level.
View population health as a dynamic and interdisciplinary approach to marshall the resources needed to assure – and sustain – community wellness and equity at the local level.
