Description
The SC Provider Toolkit was developed to fulfill a recognized need for health care providers to better manage their patients with prediabetes. It is a resource that health care teams may use as a guide to evaluate, test, treat and refer patients to in-person or online evidence-based diabetes prevention programs. The goal is to increase the number of physicians, health care providers and healthcare consumers that recognize prediabetes as a treatable condition. Objectives

At the completion of this activity, participants will be able to:

  • Define prediabetes and identify common risk factors for both prediabetes and diabetes
  • Be introduced and learn how to use the SC Provider Toolkit which can be incorporated into their practice.
  • Learn about the CDC National Diabetes Prevention Program.
  • Evaluate, test, treat and refer patients to the evidence based program (NDPP) using the SC Provider Toolkit guide
