Description
Over the past year, we have focused on using an interprofessional team to address care at the individual patient level. We’ve identified techniques, tools and resources that can be used by a variety of team members to work together to address the patient’s needs. This session will focus on using an interprofessional team to address the gaps in care for large groups of patients through the use of technology and processes to reach out to patients beyond the exam room.

At the completion of this activity, participants will be able to:

  • Identify the importance of monitoring and addressing gaps in care.
  • Describe the benefits of value-based care payment models.
  • Illustrate how the use of electronic health records and an interprofessional team can assist in providing quality care.
