Description
South Carolina ACMA invites you to join us for our Fall Conference and a great day of Education and Networking at the Florence Center in Florence, SC. In the ever-changing world of Healthcare, Case Management has become an integral piece of the puzzle. During the conference, presentations will be offered on topics that impact Healthcare and the Case Manager. Presentations include Medicare Advantage Plans and the Two-Midnight Rule, Impacting the Authorization process for Post Acute Admissions and Developing a SNF Collaborative. This year we are also excited to have representatives from SC Department of Social Services and SC Department of Disabilities and Special Needs updating and sharing how Case Managers can work collaboratively with their offices to impact the care provided to our citizens.

Objectives

At the completion of this activity, participants will be able to:

  • Know key aspects of the CMS Two-Midnight Rule.
  • Discuss the longstanding regulatory framework guidance that applies to Medicare Advantage.
  • Define what Medicare Advantage payers are and are not allowed to do.
  • Identify recourses available to providers when payers do not abide by the rules.
  • Identify the need for a means of placing unfunded, Medicaid eligible patients in the SNF.
  • Discuss how to develop and roll out a similar program.
  • Explain how to sustain the program and show the value/savings to your hospital system.
  • Describe the impact of true partnerships between hospital systems and SNFs.
  • Identify someone that may be eligible for services.
  • Explain the process of referring a person to DDSN eligibility.
  • Discuss DDSN operated waivers and services available.
  • Describe two aspects of a collaborative model to obtain skilled nursing facility prior authorization.
  • Identify one major component of an acute hospital's skilled nursing facility navigation program.
  • Analyze the use of data for process improvement.
  • Identify the law that govern APS.
  • Discuss APS philosophies.
  • Know how competency and capacity play in determining vulnerability.
  • Define what APS can and can’t do based off the law.
