Description
More than 38 million Americans (about 1 in 10 U.S. adults) have diabetes, with approximately 90 to 95% of those patients having Type 2 Diabetes. T2DM is now recognized as a chronic, complex medical condition that requires a comprehensive, interprofessional risk reduction management strategy beyond Hgb A1C reduction to improve patient outcomes. Since 1988 the American Diabetes Association (ADA) has published standards of care for patients with diabetes. The purpose of this educational activity is to highlight major updates to the 2025 ADA Standards of Care including new recommendations for GLP-1 and GIP/GLP-1 receptor agonists in patients with T2DM and chronic kidney disease (CKD) and metabolic dysfunction-associated steatotic liver disease (MASLD). Additional guideline recommendations for expanded use of continuous glucose monitoring (CGM) in patients receiving hypoglycemic agents, other than insulin, and recommendations for individualized glycemic treatment goals in older patients with multiple co-morbidities will also be discussed.

Keywords: T2DM, diabetes, pharmacy

Objectives

At the completion of this activity, participants will be able to:

  • Review the 2025 American Diabetes Association Standards of Care for Patients with Diabetes and the evidence on which they are based.
  • Discuss the results of landmark diabetes trials that established improved patient outcomes with evidence-based pharmacotherapy.
  • Describe the evolving paradigm by which the treatment of T2DM (i.e. glycemic management) has shifted to an outcomes-based approach focused on risk reduction, particularly in older patients with established CV disease, obesity, CKD, HF, MASLD, OSA, and PVD.
