Description
Join us for an interactive case-based discussion exploring how interprofessional teams support people living with HIV throughout the care continuum. Participants will hear from healthcare professionals involved in medical case management, nursing, pharmacy, and primary care about strategies to support linkage to care, retention in treatment, medication access, and patient engagement. Through real-world scenarios and collaborative discussion, this session will highlight the importance of addressing stigma, coordinating care, and leveraging community resources to improve outcomes for people living with HIV.

Objectives

At the completion of this activity, participants will be able to:

  • Describe the role of a medical case manager in linkage, retention, and re-engagement in HIV care.
  • Summarize available patient assistance programs to assist people with HIV-related medications.
  • Explain how actions by members of an interprofessional team can reduce stigma experienced by people living with HIV.
  • List ways that interprofessional collaboration improves care outcomes for people living with HIV.
